*Godparents must fill out the Godparent Information Forms (Godfather/Godmother) in the Baptism Packet. This form must be signed and sealed by the Godparent’s parish if they are not a registered member of St. Thomas More.  Godparent forms should be returned with the Baptism Packet to St. Thomas More.

*At least one godparent/sponsor must be an active and practicing Catholic in good standing with the church and meet ALL requirements on the Godmother/Godfather forms.  This includes having received all sacraments of initiation in the Catholic Church (Baptism, First Eucharist and Confirmation).
